BECOME A PATIENT
WE ARE CURRENTLY ACCEPTING NEW PATIENTS.
Follow the steps to the right to become a patient:

Please call the number listed in the contact us page or complete the inquiry form.

48 HOURS
We will reach out to you within 48 business hours to answer any questions you may have about our practice policies and fees.

FILL OUT THE FORMS
You will be sent a link that will give you access to our electronic medical record (CharmEHR) and asked to complete three documents to complete:

Consent to Treat form

Demographics form
-
Patient Intake form
Please complete all three in a timely manner as we will not be able to schedule an appointment for you until they are returned.

FORM REVIEW
We will review the forms and will reach out to you to let you know if we feel we are able to help you. If so, we will schedule an appointment with you within 1-2 weeks.
INSURANCE
We are out of network providers and do not accept insurance.
How reimbursement from your insurance works:

Prior to starting the process of becoming a patient, we recommend that you contact your insurance provider and determine whether you have out-of-network mental health benefits. If you do, ask for the percentage of the maximum allowable rate that you will be reimbursed. This will provide you with a general sense of your reimbursement for each session.

After each visit we will bill you with your card on file and then provide you with a superbill that you can then submit to your insurance provider for reimbursement.

You are responsible for the full payment after each session unless you have made arrangements otherwise.
CANCELLATIONS & LATE POLICY
The past two years have seen a dramatic increase in individuals requesting mental health care. We try to accommodate all requests for evaluation and care, and as such, we ask that you value our time as we value yours.
Appointments that are missed without at least 24 hours’ notice will be charged in full. Exceptions will be made if the appointment is rescheduled within the same week, or there are significant extenuating circumstances that prevented timely cancellation. Excessive missed appointments may cause you to be dismissed from the practice.
Late arrivals will not receive an extension of scheduled service times and will be charged the full service fee.
